PRIMARY CARE

Primary Care

✓ Care is available for children ages 3+.

✓ Appointments can be scheduled and in some instances are available as early as the same day.

✓ Primary Care appointments are available Monday - Friday from 8 AM – 5 PM EST.

✓ Option to see the same provider every time.

URGENT Rx

Urgency Care Pharmacy - Urgent Care Medications

✓ Membership covers 70 urgent medications which can be found in the member portal.

✓ Covered urgent medications will be sent to a local pharmacy. Present the urgent prescription card to a retail pharmacy to receive the covered urgent medication at no cost. The card is available to print or download from the Valorem Service portal under the “My Medications” section. *

*Medications not included in the Valorem medication list can be purchased through a local pharmacy. The cost for these medications will be incurred by the Member. If the Member has traditional insurance with a drug benefit, the Member can use their insurance card or use the Valorem Pharmacy discount card to receive additional discounts.

24/7 URGENT CARE

Urgent Care

✓ 12 virtual urgent care visits per Member per year.

✓ Care is available for children ages 3+.

✓ Appointments are available 24 hours a day, 7 days a week, 365 days a year.

MENTAL HEALTH

Mental Health Services

✓ 12 Virtual Mental Health care sessions per Member per year.

✓ Mental Health services are available for Members over the age of 12.

✓ Appointments can be scheduled within 1-5 days.

✓ Master’s level clinicians are available 24/7 for urgent needs.

✓ Sustained engagement with a Care Coordinator

✓ A direct connection to appropriate resources

MAINTENANCE Rx

Full Pharmacy - Maintenance/Mail Order Medications

✓ Over 1,000+ generic medications are covered by the membership. A list of these medications can be found by accessing the Revive member portal.

✓ Covered maintenance medication will be processed via our mail order pharmacy and delivered to the home within 3-5 business days.

✓ For the Valorem covered medications, twelve shipments per year are included free in the membership. Each additional shipment beyond twelve will require a $7.99 shipping fee. More than one prescription can be combined in a shipment.

✓ Covered medications can be transferred to our mail order pharmacy to receive the medication at no charge.

✓ Medications that are not covered in the membership will be sent to a local pharmacy. The member prescription card can then be used to receive discounts of up to 80% off. The card is available to print or download from the Valorem Service portal under the “My Medications” section. *

✓ If a provider recommends a maintenance medication that is covered, it can be processed through Valorem’s Pharmacy partner.

✓ A Pharmacist is available at 888-770-4009, Monday-Friday from 8 AM - 5 PM EST or via email.

SPECIALTY SERVICES

VISION

Vision Care

✓ Children are covered under the family program only (all ages).

✓ A member may receive 1 virtual vision acuity test each calendar year to update or renew their vision prescription.

✓If you are enrolled as a Single individual, you will receive an annual $180 eyewear allowance and an annual $180 contact lens allowance. Each allowance is broken into 4 quarters of $45 applied to your quarterly supply order, or the entire $180 if your supply order is for the entire year. 

✓ If you are enrolled as a Family, you and each member of your family have your own annual benefit allowance. 

✓ Access to daily, weekly, and monthly contact lenses from Clerio, Bausch and Lomb, Acuvue, and many other brands. 

✓ You will receive a 1-year supply of contact lenses, in some cases for as low as $10 after the discount.

✓ Eyewear and lenses start as low as $30 (after $180 annual allowance). 

✓ Eyewear frames are grouped into Affordable, Premium and Luxury styles and range from Ecko and Dickies to Kate Spade, Oakley, YSL and Prada. 

✓ Eyewear lenses are polycarbonate, with an anti-reflective coating and have UV protection, in addition to blue light lens options.

PHYSICAL THERAPY

Physical Therapy

✓ 12 virtual physical therapy visits per Member aged 18+.

✓ Children aged 16, but younger than 18 can be seen with an adult’s consent.

✓ Virtual Physical Therapy is available to help with Neck, Back, Knee, Hip, Shoulder, and Elbow pain.

✓ Appointments can be scheduled within 1-3 days and if available, same day requests can be made for the first available coach.

✓ Appointments are available Monday-Friday, 10 AM - 8 PM EST.

✓ Visits for children younger than 16 count towards the adult membership visit limit.

✓ Option to see the same coach every time.

✓ Package of additional 5 visits can be purchased for $39 if needed. Instructions are in the member portal.

It-home Labs

✓ This service consists of two parts: (1) in-home service and (2) labs through Quest Labs

✓ Both services are available as PMPM price (paid by employer) and pay per use (paid by member)

✓ Our PMPM price of $26 for single and $46 for family (up to 7) includes Quest's Basic Health Profile lab (59+) indicators once per year. If second test is recommended by a provider, we can do it twice at no extra charge.
