PREVENTIVE SERVICE
With your plan, there are no network restrictions. You can visit any provider or facility of your choice.
To determine if your medical need is considered preventive, please see the following website for a list of current preventive services: Preventive health services | HealthCare.gov  If your desired service is not on this list, it will not be considered and billed as preventive.  Please visit the Non-Preventive Services Tab. 
When you arrive at your provider:
1.Show your Planstin insurance card (If your provider is not familiar with Planstin, have them call 888-920-7526).
2.Receive your test/care.
3.Pay any fees with HSA card.

OTHER NON-PREVENTIVE SERVICES
With your plan, there are no network restrictions. You can visit any provider or facility of your choice.

Most nonemergency sharing requests, such as surgical procedures, should be submitted prior to the date of service. Members may contact Zion HealthShare directly during business hours for help with this process. 888-920-9466 or member@zionhealthshare.org

  2. Do not show your Planstin card.
  3. Pay any fees with HSA card.
    1. Note: If hospitalization is required or if you believe that your cost will exceed $1,000, REQUEST AN ITEMIZED RECEIPT when receiving services and then call this number at your earliest convenience to help initiate your Zion HealthShare process-(888) 399-0017
    2. Be sure to keep any receipts or invoices you receive from the provider.
