In the event that your provider says they do not recognize or accept Planstin, or if you want the best price on your vision care, present your self to ANY licensed eye care provider (regardless of networks) as a SELF-PAY CUSTOMER, get the discounted self-pay rate, and submit an itemized receipt for reimbursement to claims@planstin.com. Because the provider won’t have to bill insurance you will receive the best rate they offer and you will be reimbursed by Planstin. You can also present your Planstin Vision Insurance Card and, if they accept the Planstin Vision, they will bill the insurance but you will not get the self-pay rate.
When you arrive at your provider:
1.Show your Planstin Eye/Vision card-this card will be separate from your Medical card.
2.Receive your vision care e.g., eye exams, contact evaluation, lenses, etc.
3.Pay any required amounts or additional fees. If you have an HSA card, you are welcome to use it.
