Standard Dental Plan:

Your Planstin Vision plan includes low copays for an eye health exam, and an allowance for frames or contacts.  All copays will be waived when you receive services at wholesale clubs.  Your plan will work with any licensed provider.  In the event your provider is unable to bill the plan, you may submit an itemized receipt for reimbursement to member@planstin.com

When you arrive at your provider:

1.Show your Planstin Eye/Vision card-this card will be separate from your Medical card.

2.Receive your vision care e.g., eye exams, contact evaluation, lenses, etc. 

3.Pay any required amounts or additional fees. If you have an HSA card, you are welcome to use it.

Note: Your plan will pay up to $150 per member, per plan year for vision services (e.g., exams, and refractions).  You plan will pay up to $150 per member, per plan year for vision equipment (e.g., frames, lenses, and contacts).
