If you choose not to use Care Coordination prior to receiving care, your visit/treatment will be subject to applicable copays, deductibles, out-of-pocket maximums and, even though unlikely, potential balance bills.  You may also elect to present as a self-pay customer to take advantage of the self-pay rates that are not billed through insurance.  

When you arrive your provider: (Note - with your plan, there are not network restrictions.  Care Coordination can assist you in finding the most cost-effective care.)

1.  Follow all instructions that you received from Care Coordination

2.  Show your Planstin Card OR

3.  Tell the front desk you have insurance but are choosing to present as a self-pay customer

4.  Receive your test/care

5.  Based on your decisions above, pay any copays, costs or fees with virtual card from Planstin or with your own funds
