If you choose not to use Care Coordination prior to receiving care, your visit/treatment will be subject to applicable deductibles, coinsurance, out-of-pocket maximums and, even though unlikely, potential balance bills.

When you arrive at your provider: (Note-with your plan, there are no network restrictions.  Care Coordination will assist you in finding the most cost-effective care.)

1.Follow all instructions that you received from Care Coordination.

2.Receive your test/care.

3.Pay any required copays, coinsurance or additional fees with the virtual card provided by Planstin.
