Your Standard Dental Plan includes 100% preventive dental care and a PPO, Premier and Non-Participating plan options.  Coverage tiers include preventive, basic, major services.  If you need assistance, or have any questions, feel free to contact a Delta Dental Benefits Advocate at (800) 489-3580

When you arrive at your provider:

1. Show your Delta Dental card – this will be separate from your Planstin Medical card
2. Receive your dental care e.g., cleaning, x-rays, fillings, etc.
3. Pay any required copays, coinsurance or additional fees. If you have an HSA card, you are welcome to use it.
